The National Forum for Heart Disease and Stroke Prevention (National Forum) seeks funding from the National Heart, Lung, and Blood Institute (NHLBI) in support of “Championing a Heart-Healthy America: Uniting Against the Leading Cause of Death”. The 2025 National Forum Annual Meeting, our 23rd event, will bring together health changemakers to improve heart health and close the U.S. life expectancy gap by spotlighting strategies proven to prevent cardiovascular and metabolic disease across the lifespan. We will explore how targeted interventions can create significant positive changes. The 2025 Annual Meeting is open to all populations and will elevate prevention and management approaches to cardiometabolic health, assessing the strengths of guideline-based care and the ideal conditions to transform research results into evidence-based practice. The specific aims/meeting objectives are to: Engage health professionals, health care decision-makers, and researchers in a conversation on the challenges posed in preventing cardiometabolic diseases, which remain among the leading causes of death despite intensive research, scientific innovation and an expansive evidence base. Continue exploring high impact opportunities to improve health outcomes. Follow-up on and present participant achievements based on previous conferences. The National Forum’s impact in previous meetings has been to bring together stakeholders who then take their perspectives and action items back to their organizations to enact practical changes to enhance health outcomes as relates to cardiovascular health. In line with previous convenings, we expect 325 participants at our Annual Meeting 2025 from across all health sectors. Follow-up activities will include on-demand video, a summary of recommendations and a digital one pager with links to all conference materials and references. The National Forum will also track actions or policies that were implemented because of the strategies and information presented at the meeting to create a national map of impact.
